According to the Council of Europe Annual Penal Statistics for 2018[1], Czechia is on the 6th place with the highest prison populations among Europe countries.  There are 208,8 inmates per 100,000 inhabitants. In January 2018, there was approximately 22 159 inmates in Czechia prisons[2]. Approximately 11% of the prisoner’s population serve their sentence for the drug law offences (2 431 prisoners in 2017)[3]. The average imprisonment term imposed by the courts for the drug law offences is approximately 4 years[4]. HIV prevalence in Czechia prisons is around 2,4%[5]. Speaking about drug use responses in prison, it should be mentioned, that OST exists, but it is very limited (from 10 authorized prisons to provide OST, only 6 are doing so). Total amount of OST patients in 2017 was 63 and the average length of the OST in prisons is 7,5 months. Inclusion in OST in a prison is subject to prior participation in the programme before being taken into custody or prior to the onset of imprisonment[6]. ART is available, NSP doesn’t exists in prison settings. According to the SPACE I 2018 survey, the average amount spent per day for the detention of one inmate in Czechia prisons is 46,50 Eur/per day, which is 16 972,50 Eur./ per year/ per inmate.

UNAIDS data 2018 report states, that there is around 42 800 people who inject drugs[7]. HIV prevalence among PWID is one of the lowest in Czechia, it’s 0,2%[8]. The costs of NSP services are 122,00 Eur/ per person/ per year and OST – approximately 1 978,30 Eur./ per person/ per year.[9] The minimum unemployment benefit sum is 132,00 Eur./ per person/ per month (which is 1 584,00 Eur/ per person/ per year)[10].

So if the person would need OST, NSP and unemployment benefit, it would cost for the state around 3 684,30 Eur./ per person/ per year.

The health and social services costs are more than 4,5 times less than incarceration costs.
